4. Enter the information requested.
- User Name
- Email Address
- Password (2 times)
- Confirmation Code as shown on YOUR Screen
- VIP Code - enter the 3 letter name of this organization here. (ALL CAPS PLEASE)

5. Scroll to the bottom of the page.
Click on Submit.
- All other buttons and fields can be ignored.
